Parents who need to enroll a new, returning or intra-district student may enroll online or download the three forms below (Enrollment, Residency Survey, Home Language Survey.)  Return the three forms with with copies of birth certificate, immunization records, and proof of residency. Mail to 3394 Woodland Dr. Mariposa, CA 95338, return to School Office, or scan completed forms and email to  mtate@mcusd.org
