New and returning students will need the following items to enroll at MCHS:
- Enrollment Form (online or fillable PDF below or paper copy in counseling office)
- Copy of Birth Certificate
- Copy of Immunization Records
- Transcript from last enrolled school
- Drop Grades, if enrolling during the semester
- Copy of 504 Plan or IEP, if applicable
- Caregiver Affidavit, if applicable
