Proof of the vaccine must include: Student’s name, date of birth, vaccine name, date vaccine administered and name of the clinic, office, or doctor administering the vaccine. If parents have not provided proof to their school by the first day of school your student will NOT be allowed to attend.
Immunization Requirements
Changes to California school immunization medical exemption requirements, effective January 1, 2021
Two new California laws (SB 276 and 714) effective January 1, 2021 affect how school districts accept medical exemptions for required immunizations. Beginning in January 2021 all medical exemptions will be submitted electronically into the California Immunization Registry for Medical Exemptions (CAIR-ME) and school districts can no longer accept any other form of documentation for medical exemptions. Please consult your healthcare provider for any questions regarding medical exemptions.
