New and returning students will need the following items to enroll at MCHS:

• Enrollment Form (online or fillable PDF below or paper copy in counseling office)
• Copy of Birth Certificate
• Copy of Immunization Records
• Transcript from last enrolled school
• Drop Grades, if enrolling during the semester
• Copy of 504 Plan or IEP, if applicable
• Caregiver Affidavit, if applicable
